Caregiver Qualifications:

*Written references of names, addresses, and phone numbers of clients, registries, or agencies

*Florida state license for RN, LPN, and CNA

*HHA certificate demonstrating knowledge in the curriculum and hours mandated by the State of Florida (75 hours)

*Proof of a physical examination by a Physician, Physician’s Assistant, ARNP, or Registered Nurse under supervision of Licensed Physician in the last six months with a physician statement that states: Free from any communicable diseases

*AHCA Level 2 background fingerprint screening

*Driver’s License

*Car insurance

*Reliable Transportation

*Social Security Card

*CPR Card (Must be from American Heart Association, Red Cross or Health & Safety Institute)

*HIV/OSHA Certification 

*Assistance with Self Administration of Medication Certificate

*Proof of Residency or Citizenship: Passport, Permanent Resident Card, Birth Certificate, Voter Registration Card, or Certificate of Naturalization

*Professional Liability Insurance 

*2-3 Professional References
